If you do not want to install Insane's HUD then you can use the configs that I posted at the following link to decrease the size and remove the death notice background. Link with commands to change the death notice size.

Minidumps are saved to C:\Windows\Minidump\ unless specified otherwise. See if you have any in there (if not then minidumps are not turned on) and then post the most recent file (as long as it occurred in the time frame of the issue).
